Account creation or access to the Services constitutes agreement to these Terms. You represent that you have full authority to enter into this agreement and that all information you provide is true and accurate. You are responsible for maintaining the confidentiality of your account credentials.
Unauthorized or illegal use of the Application is strictly prohibited and may result in immediate termination of your account.
Form500mg provides an AI-powered form builder platform that enables users to create, distribute, and analyze forms, surveys, and questionnaires. Subject to these Terms, we grant you a limited, non-exclusive, non-transferable, revocable license to access and use the Services for your internal business or personal use.
AI Features: Our platform utilizes artificial intelligence and machine learning features to assist with form creation. AI-generated outputs are probabilistic in nature, and you are responsible for reviewing and verifying all AI-generated content before use.
We may suspend or restrict access to the Services: (a) immediately for security risks, suspected fraud, or material breach; (b) after 15-day written notice for curable breaches; or (c) for non-payment of applicable fees.
Form500mg uses Amazon Simple Email Service (AWS SES) to send transactional email communications. By using our Services, you acknowledge and consent to receiving the following types of emails:
When a respondent submits a response to your form, we send a notification email to the form creator informing them of the new submission. These emails include the form name, submission timestamp, and a link to view the response.
When a respondent completes a form, they may receive a one-time confirmation email with a copy of their submitted responses. This is only sent when the form creator has enabled the "send response copy" feature and the respondent has provided their email address as part of the form.
Emails related to account verification, password resets, billing, and important service updates.
We do not send marketing or promotional emails unless you have explicitly opted in. All transactional emails are triggered solely by user-initiated actions on the platform.

Form500mg retains all rights, title, and interest in and to the Application, Services, software, and all related intellectual property. Nothing in these Terms transfers any ownership rights to you.
You retain ownership of all content and data you create, upload, or collect through the Services ("Customer Data"). You grant Form500mg a limited, non-exclusive license to process your Customer Data solely for the purpose of providing the Services.
Any feedback, suggestions, or ideas you provide about the Services may be used by Form500mg without any obligation or compensation to you.
Your use of the Services is also governed by our Privacy Policy and Data Processing Agreement. In the event of a conflict between these Terms and the Privacy Policy or DPA regarding data handling, the Privacy Policy or DPA shall control.
You are solely responsible for ensuring that your use of the Services, including any forms you create and data you collect, complies with all applicable data protection laws and regulations (including GDPR, CCPA, and others).
Form500mg offers multiple subscription plans (Free, Founders, Enterprise). Plan details, including features, credit allowances, and pricing, are available on our pricing page. All fees are non-refundable unless otherwise stated.
We reserve the right to modify pricing with 30 days' advance notice. Continued use after a pricing change constitutes acceptance of the new pricing.
These Terms are effective upon your first use of the Services and remain in effect until terminated. You may terminate your account at any time through your account settings.
We may terminate or suspend your account: (a) for material breach of these Terms; (b) if required by law; or (c) if you have not used the Services for an extended period. Upon termination, your right to access the Services ceases immediately.
We will retain your data for 30 days after termination to allow you to export it. After this period, your data will be permanently deleted.
You agree to indemnify and hold harmless Form500mg, its officers, directors, employees, and agents from any claims, damages, losses, or expenses (including reasonable attorney's fees) arising from: (a) your use of the Services; (b) your violation of these Terms; (c) your violation of any applicable law; or (d) any data collected through forms you create.
The Services are provided "AS IS" and "AS AVAILABLE" without warranties of any kind, either express or implied, including but not limited to warranties of merchantability, fitness for a particular purpose, or non-infringement.
Form500mg does not warrant that the Services will be uninterrupted, error-free, or secure, or that any defects will be corrected. AI-generated content is provided without any guarantee of accuracy or completeness.
To the maximum extent permitted by law, Form500mg shall not be liable for any indirect, incidental, special, consequential, or punitive damages, including loss of data, profits, or business opportunities, arising from your use of the Services.
Form500mg's total aggregate liability for any claims arising under these Terms shall not exceed the total fees paid by you to Form500mg during the twelve (12) months preceding the claim.
These Terms shall be governed by and construed in accordance with the laws of the State of Delaware, United States, without regard to conflict of law principles.
Any disputes arising from these Terms shall first be attempted to be resolved through good-faith negotiation. If unresolved, disputes shall be submitted to binding arbitration in accordance with the rules of the American Arbitration Association (AAA).
If any provision of these Terms is found unenforceable, the remaining provisions shall remain in full force and effect.
These Terms, together with the Privacy Policy and DPA, constitute the entire agreement between you and Form500mg.
Form500mg may assign its rights and obligations under these Terms without notice. You may not assign your rights without our written consent.
